Award Description: IDS funds support STEEMCC's activities to enhance case management services and assure increased access to primary, preventive care for the target population with a special focus on the pediatric and adult high risk population.
Project Description: Existing medical services were expanded with the addition of an Internist and LPN/Case Manager. This is anticipated to continue primarily since the target population of uninsured or underinsured individuals have access to a regular source of care and opportunity for improvement in health outcomes. Improvements in an integrated management information system to enhance data collection, patient tracking and clinical records management are on-going through services provided by an IT technician. Training for operation and utilization of MDServe database for relevant staff was conducted in September 2009.
Jobs Summary: An Internal Medicine physician was recruited and hired at 0.50 FTE to provide primary, preventive care since June 2009. This has increased accessibility and availability of services for new and returning adult patients seeking appointments and decreased lengthy waiting periods due to lack of physician coverage. A total of 309 patients have received comprehensive medical consultation and health education. A LPN/Case Manager was recruited in September 2009. This position is funded at 1.0 FTE. In addition to the primary responsibility of providing case management and care coordination services for the adult high-risk or at-risk population, this individual will be responsible for collection of information relative to assessing the number of patients who are uninsured, on sliding fee scale or are covered by the VI Department of Health's Medical Assistance Program. The IT technican was retained at 0.25 FTE and will continue to provide services as needed for management of the existing and proposed data management and electronic health record systems.
