Award Description: The project will provide up to twenty four months of transitional housing and voluntary supportive services with the goal of assisting homeless women and children, fleeing domestic violence, dating violence, stalking or sexual assault, to achieve safety and stability in their lives. Expected outcomes are increased personal safety and residential housing stability of the participants, increased income/self-sufficiency and greater self-determination. Deliverables include the provisions of: 1) placement of family in safe, stable transitional housing per signed leases, service and safety plans per presence of safety/service plans with short and long term goals in client files, case managment per weekly case notes in client file, aftercare services in permanent housing per case documentation; 2) employment per paystubs, entitlement benefits per provider statement, educational/training accomplishments per certifications in client file; 3) improved life skill knowledge per Daniel Memorial pre and post test scores, participation in life skills classes/enrichment social opportunities per documentation of dates and types of sessions attended, DV/SA counseling sessions per partner documentation of client participation.
