Award Description: The City of Modesto establishd a sub-review committee with the local Continuum of Care, which included representatives from Community Service Agency that administers the Temporary Assistance to Needy Families as well as other federal funding. The group determined that there were three main areas to target funding, citizens with physical disabilities and other chronic health issues including HIV/AIDS, person’s with mental health and substance abuse issues, and individuals and families who are homeless or at risk of becoming homeless. The consensus of the group was to utilize existing organizations that have the experience, capacity and existing programs to be able to expedite the funding and make an impact in serving the community. Community Housing and Shelter Services will provide comprehensive set of services to low/moderate income households to prevent homelessness or rapidly to obtain permanent housing. Stanislaus Community Assistance Project will address the needs of households that have disabilities including chronic health issues including HIV/AIDS. Both agencies are established non-profit organizations within the community that have existing programs to manage clients with housing challenges.
Project Description: During the first quarter reporting a lot of time was spent on administrative activities and program development. The City of Modesto has only had the HPRP Grant agreement for approximately one month at this point and therefore there aren't any quantitative results at this time. The two sub-grantees have executed agreements with the City of Modesto and are actively working on outreach.
