Award Description: ARRA - Increase Services to Health Centers
Project Description: The addition of a clinic practice manager/coordinator has provided the ability to monitor patient flow on a consistent basis and at a high level. The addition of case managers and eligibility workers has provided a link between the community and the health center to increase access to services for residents and provide a continuum of care for patients. These individuals serve as patient advocates to overcome the most common barriers such as lack of knowledge of available resources, fear, lack of insurance, lack of transportation and an inability to communicate effectively.
