Award Description: Canton Community Clinic proposes to use Increased Demand for Services funds to increase access to medical services to address the identifies community and target population health care needs. Canton Community Clinic provides servies at times that meet the needs of the population including evening hours. This project will increase provider hours during CCC' evening clinic hours each week. Many community members have recently lost manufacturing jobs and their associated health insurance, and sick, vactio, and personal leave benefits. Those that have found employment have moved to lower paying mobs in the service ind8stry with no insurance and little leave time available. The addition of provider time during clinic night hours will remove a significan barrier to care for the recently unemployed and other disadvantaged members of the community. Canton Community Clinic employes a patient advocate to provide outreach services to patients. This outreach may include information, transportation, housing, employment and nutrition services as appropriate. However CCC is aware that addressing the health care needs of the newly emerging unemployed populations will require further development of intensive outreach activities and communications planning to reach and inform them about CCC services.

Project Description: Increase hours of service for nurse practitioners. Hire additional outreach/support staff.

Jobs Summary: We have increased the hours of service provided by our nurse practitioners to provide an additional day of availability as well as additional evening hours. Additional time LPN time has also been created in support for these providers. The evening hours can be particularly important for patients who have limited time away from employment during the day, or have other transportation or child care issues to arrange in order to seek medical attention. We have hired a patient outreach coordinator to assist patients with assessing their financial status and/or seeking assistance that might be available. We have also added an additional receptionist to support better procedures for greeting and checking in patients, responding to phone calls, and monitoring activities in the lobby.
