Award Description: The IDS project for the Health Care Center for the Homeless, Inc. (HCCH) involves retaining two key positions at the Center; a primary care provider and the Health Navigator. The primary care provider will continue to provide primary health care services to the homeless population of Orange, Osceola, and Seminole Counties in East Central Florida. While HCCH will not be adding new employees the Center has initiated several process improvement strategies aimed at increasing the efficiency of the Centers’ operations. These increased efficiencies will expand the Centers’ capacity for daily medical encounters. An example of these strategies is the 'Dr. of the Day initiative. This initiative rearranged the scheduling process of the center and identifies one provider a week to see nothing but walk in patients. This change was made in response to the increasing number of new uninsured homeless patients who presenting at the center to be seen without an appointment as of the last quarter of 2008. The Health Navigator will continue to interview every homeless patient that presents at the ED, schedule their follow up appointment with HCCH and arrange transportation if necessary. The Health Navigator will also follow up with them to make sure that they keep their appointment and address any barriers that they may have encountered when attempting to access services.
