Award Description: Community Health Connection will hire a Family Practice Physician Assistant/Nurse Practitioner and a Medical Assistant 5-2009 creating two new jobs. The new provider will see patients throughout the lifespan providing a primary medical home to the patients that are in need of health care. We will seek to hire Spanish speaking staff as 65% of the clinic's 2008 patients were better served in another language (UDS 2008). In addition, these patients will have access to the mental health, prenatal care and enabling services available at the health center. This proposed IDS project is within the approved scope of CHC. This new team will work three 12 hour shifts and 4 hours on Saturday expanding clinic hours by 9.5 hours per week. The current clinic support staff will adjust their shifts to accommodate the new hours and provide coverage. These new positions and increased hours will provide increased capacity for CHC to serve 2813 patients with 6120 projected encounters over the project period. Through the Access and Redesign Health Disparities Collaborative CHC has been able to provide same day/next day appointments for their patients with the 3rd next available appointment today. T his open access scheduling will provide these patients with the care they need today or tomorrow. 51% of CHC patient hear about our services from word of mouth. To inform these patients and the public of our new hours and increased capacity we will develop a flyer and posters to have available in the clinic for our clients to take and distribute. The flyers will also be distributed throughout the community at other helping agencies, stores, Laundromat, eating establishments, churches, schools and places where people affected by the downturn in the economy and in need of health care would potentially visit (unemployment office, Salvation Army, and hospital emergency rooms). Jobs Summary: Nurse Practitioner: The Family Nurse Practitioner is an integral member of the interdisciplinary health team of Community Health Connection, Inc. The FNP provides primary care for clients, and manages discharge for inpatient hospital clients in collaboration with Hospitalists and other center physicians. The FNP also serves as a preceptor for other health professions, students and residents. There may be rare situations when the FNP will manage hospitalized patients Medical Assistant: The medical assistant is a member of the interdisciplinary medical team of the health center. She is responsible for intake work with clients as they enter the clinical area. She is a liaison between the front desk staff, the provider staff along with the nurses in the provision of care.
