Award Description: The overall goal of HCHP is to improve the health care status of homeless persons in the New Orleans area by providing a holistic, multi-disciplinary team approach to problems facing homeless individuals and their families. The New Orleans area is facing an unprecedented mental health crisis and the city has no way to deal with it. The number of public mental health facilities and community outreach centers have dramatically decreased, leaving the mentally ill without medication and monitoring. In order to meet these needs for our population, HCHP is expanding mental health services and further integrating addiction and case management services into our service delivery model. The integration of mental health, addiction services and case management available at one location has proved to be a best practice with evidenced-base. Therefore we are hiring 2 FTE's a social worker and substance abuse counselor and increasing psychiatrist services by .2 FTE. We will also provide onsite HIV testing which includes pre and post counseling. HCHP will provide medical outreach utilizing a renovated medical mobile unit in two different locations, at one of the locations services will be provided during non traditional hours.The additional services, staff and hours of operation will address barriers to care by increasing capacity to see more patients. We anticipate increasing new patients by 1200.
Project Description: We have hired our casemanager and she will begin work the middle of October; she will provide out reach and intense case management, this will enhance our efforts In the retention of patients with serious and complex behavioral health disorders. We have experienced great success with our street outreach efforts and extended hours. At these locations, we are serving approximately 20 persons per week with about 75% neing new patients to the clinic.
