Award Description: Homeless Prevention and Rapid Re-Housing Program, aka Project Action, a collaboration between the City of Tucson and Pima County that provides case management and financial assistance in the form of rent, utility, moving, storage and motel vouchers. The program provides credit repair counseling and legal aid. The service targets clients who would be homeless with out this support or to move clients from homelessness and stay housed with the support.
Project Description: From June 23, 2009 to August 4, 2009 Project Action held a series of public meetings to determine the design of the program with the HPRP funds that would create a collaboration between the City of Tucson and Pima County. RFPs were released and contracts were executed with 5 subgrantees who will assist in the implimentation of Project Action. It is anticipated that 400 households will be served from to 3 months up to 18 months each. Success is defined as households served will stay housed for at least six months, mainstream referrals will be effective. Opportunity for new collaborations and partnerships. Create a new paradigm for effective problem solving and accountability through an oversight committee. Best practices will be documented and sustained.
Jobs Summary: Resource Specialist: Determine eligibility, develop case plan from self sufficiency matrix, determine immediate and long term financial assestance, provide follow- up and community resources, review case plan and perform exit review. Provide documents and financial assistance request to Contract Specialists Contract Specialist: Review needs assessments, eligibility and documentation varification developed by Resouce Specilist.
