Award Description: Santa Monica's Homelessness Prevention and Rapid Re-Housing program (HPRP) targets low-income, at-risk, housed Santa Monica residents who are experiencing a housing crisis. Services are designed to help the household maintain housing, prevent homelessness and stabilize. Case management, housing stabilization and/or relocation, legal services, finanical assistance and administrative services are available.
Project Description: Services will prevent eviction and facilitate rapid re-housing for low-income, at-risk, housed Santa Monica households. Program services will also connect households to resources that support long-term housing stability. In Year 1, 50 families will complete a Family Housing and Service Plan and receive direct services and linkages to services and supports required to maintain housing stability. In Year 1, 40 families, or 80% of the 50 families to be enrolled, will receive one or more types of finanical assitance. Of these 40, 95% or 38, will stabilize their housing crisis and avoid immediate homelessness.
