Award Description: The IDS (job investments) grant will add 9.5 FTEs, of which 3.50 will be retained jobs and 6.0 will be new jobs. Unduplicated Geriatric patients will increase by 1600 with an additional 5160 visits.This job investment will be the impetus of getting the over 65 year old population quality health care.
Project Description: The proposal of the Greater New Bedford Community Health Center in its culmination will have 1600 new Geriatric patients 5160 visits. At the end of the second reporting quarter, 9/30/09, we have 649 new Geriatric patients and 1840 visits.
Jobs Summary: 1.00 RN to provide medical support for the Geriatric patients. 2.00 Outreach Workers to assist in all enabling services associated with primary care and referrals. 1.00 Eligibility Assistant Worker to assist Geriatric patients with their insurance coverage. (Total jobs reported: 8)
