Award Description: The CIP will provide needed renovation Heartland Community Health Clinic, and ensure that infrastructure network equipment is updated to meet the many challenges and changes in HCHC service area. HCHC serves the needs of Peoria County. It has grown to three main medical sites. HCHC has a number of renovation projects that will increase the usable life of its buildings, upgrade furnishings, clinical equipment and add clinical space. The projects will improve the appearance and safety of HCHC?s properties, the seating in waiting rooms and exam room equipment. Quality of care will be improved by making additional and/or improved space available to increase staff efficiency and productivity.
Project Description: Renovation of: Heartland Community Health Clinic at Carver; infrastructure upgrades in readiness for Electronic Health Records.
Infrastructure Description: 1) Fund portion of initial Electronic Health Records readiness; 2) Fund technical assistance for HER planning and implementation; 3) Expand and renovate facilty space to increase access to care at the HCHC Carver Clinic.
