Award Description: The goal of this application is to improve West Side Community Health Services (WSCHS)' effectiveness, efficiency, quality of care, and patient outcomes through a multi-part capital-improvement project: a) Implementation of a collaborative Electronic Health Record (EHR) in partnership with two other Community Health Centers and a Health Center-Controlled Network, which will improve the quality of care, enhance patient safety, improve patient and clinical satisfaction, increase productivity, and enhance revenue. This project builds on two years of collaborative planning and readiness assessment, and will help WSCHS meet the state of Minnesota requirement that all health providers in the state have a fully functional EHR in place by 2015; b) acquisition and installation of a telepharmacy system, which will improve patient safety and reduce adverse events, improve access to community-based clinical pharmacy services, increase the cost-effectiveness of WSCHS' pharmacy services, and improve patient health outcomes through an innovative technology that will be used initially between the La Clinica and East Side sites, but potentially with other sites as well, including other Health Centers that do not currently have on-site pharmacies; c) acquisition of a new digital radiography system, which will increase the cost-effectiveness of WSCHS' radiology and related clinical services, increase care coordination across WSCHS, both internally and with external partners such as hospitals and specialists, increase the impact and effectiveness of WSCHS' new EHR system; and improve overall patient health outcomes by allowing WSCHS to upgrade the current digital xray system now in use at La Clinica and bring digital xray for the first time to our East Side Family Clinic site; and d) acquisition and installation of a digital dental xray system at our two dental sites, which will increase the cost-effectiveness of WSCHS' dental services, increase care coordination across WSCHS, both internally and with external partners; and improve overall dental patient outcome through upgraded x-ray equipment, including a panoramic system, and new capability to share x-ray records electronically, including across across sites.

Project Completion–12/10 Challenges • Workflow analysis–documenting paper processes, creating new electronic ones • Gaining consensus among all clinics • Clinician time availability • Communication–A plan is needed • Technology–Staff adoption of new technology • Pre-loading data • Change management Overall, the project is on track. The Go Live date is extended, but the completion date is not. WSCHS recognized a need for more clinical content planning. Consensus takes longer, but the trade-off is greater user acceptance and a system that meets needs. WSCHS Digital X-ray Upgrades & Telepharmacy Technology Digital X-ray Upgrades: WSCHS ordered 2 digital x-ray machines. La Clinica already owned/operated an x-ray machine and was able to trade and upgrade. A new machine was purchased for ESFC. 35% of the cost was paid as a down payment. Delivery is scheduled. Technology compatibility issues with new equipment and current computers/printers is being assessed. Digital Dental X-ray System: Cabling/electrical work was completed; equipment was ordered, delivered, & installed. The vendor is now testing equipment/software. Staff training is occurring. Band width is being expanded to allow for faster data transmission. Telepharmacy Technology: This project has not yet begun pending WSCHS’ FIP grant application outcome. If awarded, a new ESFC will be constructed and may not require the technology proposed. WSCHS will determine how to proceed next quarter.
