Award Description: Increased Demand for Services (IDS) Grant: American Recovery and Reinvestment Act
Project Description: Curtis V. Cooper Primary Health Care hired nurse practitioner in May 2009, a clerk in July 2009, and a RN case manager in August 2009. During the third quarter ending September 2009, in addition to the nurse practitioner hired previously, a clerk was hired in July 2009 and a RN case manager was hired in August 2009. After working for several weeks the clerk hired was unable to fulfill the work hour requirements and subsequently resigned. Therefore, there are three remaining positions to be filled a clerk position, a LPN position and an additional RN case manager position. The positions of LPN and RN case manager that was not filled ending the third quarter has been filled as of the submission date of this report. The clerk position has yet to be filled. During the calendar year 2008 Curtis V. Cooper Primary Health Care (CVCPHC) provided generated 41,042 medical visits and served more than 15,900 medical patients. During the 12 month period beginning July 2008 until June 30, 2009 CVCPHC generated 37,250 medical visits and served 15, 172 patients. Beginning October 2008 ending September 2009 CVCPHC generated 40,488 medical visits and served approximately 16,202 patients. CVCPHC generated 9,649 medical visits during the second quarter ending June 30, 2009 and served more than 7,271 patients. In comparison there were 10,405 medical visits generated during the third quarter ending September 30, 2009 and 9,445 patients were served. Medical visits increased by 1,008 or 9% when comparing the second quarter and third quarter activities of 2009. There was an increase in patients by 2,174 or approximately 30% during the same period. CVCPHC is unable toe report the number of new uninsured patient served during the report period. The existing practice management system is unable to capture and report detail patient and visit data necessary to provide the information requested for the specific report periods required.
Jobs Summary: Nurse Practioner @ .42 FTE to provide primary health care services, .28 FTE clerk receive and enroll patients in support of the additional provider (nurse practitioner) and a .15 FTE RN case manager to assist patients in getting their medication rewritten by the primary care provider, to assist providing in completing written orders for patient referrals, assist patients and providers by following up on patient messages and telephone inquires, etc. The clerical position is currently vacant due to employee resignation, therefore, there are three positions that will be filled during the fourth quarter ending December 31, 2009.
