Award Description: Recovery Act Health Center Cluster Program, New Access Point to provided health care for the homeless program.
Project Description: To provide primary health care services to the vulnerable homeless populations of Leon County. All staff has been hired and project is fully implemented with outreach workers penetrating the community. Mobile modular Unit is completely operational. The ARNP recorded 136 new users equating to 136 visits of which 122 patients were uninsured. The Case Manager recorded 87 encounters for social support services.
