Award Description: The proposed Phase II EHR Development Project will allow expansion and dissemination of Health Choice Network?s multi-market, sustainable business model for adoption of Electronic Health Records (EHR) in CHCs. The goal of the proposed Project is to harness the power of EHR towards assisting member CHCs in improving their delivery of health care and, thereby, improving health outcomes for the patients they serve. Specifically, the Project will drive the implementation of a shared clinical information system (EHR) to support quality improvement efforts at member CHCs through integrated clinical benchmarking systems, population-based reporting, and development point of care decision support tools for both provider and patients. The Health Choice Network collaboration will involve eleven CHCs located in four states: Kansas, New Mexico, Hawaii, and Florida. Together the collaborators for the Phase II EHR Development Project will impact nearly 164,000 patients and 159 medical and 22 dental providers. The collective impact of the Health Choice Network Phase I (ICT) and Phase II EHR Development projects will integrate 325 EHR and 64 EOHR providers on a common EHR/EOHR platform. As an early pioneer in the development of Health Information Technology (HIT) systems for safety-net providers, Health Choice Network has developed a time-tested business model for the successful incorporation of HIT/EHR in a CHC setting to improve business and clinical operations. The Health Choice Network model is based on four sound IT implementation principles customized to address the CHC environment: 1) Extensive KNOWLEDGE of CHC operations and patient mix; 2) Value ?added TECHNOLOGY; 3) Performance and Quality-driven PROCESSES; and 4) Experienced and uniquely qualified STAFF.
Project Description: EHR Implementation is complete or has been initiated in 7 of the 9 CHCs. The original scope projected bringing a total of 159 providers live onto an EHR system. Currently we are on track to exceed this goal and bring 258 providers live on EHR
