Award Description: Goal 1. Prevent people from becoming homeless. Prevention requires extensive community outreach to identify people at risk of becoming homeless. It also requires good targeting of prevention resource to ensure that resources reach people most likely to become homeless. The Plan includes objectives related to outreach, targeting, and effectiveness. A. Prevention Outreach ? An estimated 2,000 individuals are homeless in Kansas according to HUD?s definition of homelessness. (This estimate is from the 2007 Kansas ?Point-In-Time-Count.?) These individuals will be targeted to receive a preliminary housing assessment as the result of targeted prevention outreach. B. Targeting ? Risk assessment criteria will target individuals who would likely to become homeless without the assistance provided. Homeless data from HMIS, local city and county surveys and any other source of data will assist in determining who those individuals might be. Focus will be upon living situations, income level, level of disability, level of supports, education, etc. C. Effectiveness ? How effective are the programs to avoid homelessness? The homeless prevention programs will measure the effectiveness of their programs by providing just enough assistance to prevent homelessness. Resources will then serve more people and prevent more episodes of homelessness. KHRC will provide guidelines on how to address rental assistance and flexible financial assistance and programs will then need to ensure that these resources are used efficiently. Program will be expected to measure the effectiveness of their programs. To do so will require them to take into account for varying levels of risk to ensure they have served people at highest risk of being homeless. Goal 1 Measures: ? Number of individuals or households identified as having a housing problem that warrants a referral to a homeless prevention program. ? Number and percentage of people served who are at high risk of homelessness (using the ?high-risk ?status). ? Cost per household assisted. Goal 2: Divert people applying for shelter, when appropriate, into other housing. Whenever people apply for shelter, they will be automatically assessed for the possibility or remaining in their current housing or staying in alternative accommodations, even for a short time while more stable housing is identified. This would involve flexible financial assistance to be used for a variety of needs including payments to family members who agree to house a person or family, if appropriate, until a more permanent solution can be secured. Goal 2 Measures: ? Number and percentage of households who apply for shelter or other homeless assistance who are assessed for diversion ? Number and percentage of households that are successfully diverted from homelessness ? Number and percentage of households who are diverted who become homeless within 12 months ? Cost per household assisted Goal 3: Help people who become homeless to quickly move into permanent housing. Most people who are homeless can exit this state quickly with assistance. The plan addresses how people will be assessed within a few days of entering shelter for their strengths, resources and barriers to exiting homelessness. Focus will be upon credit problems, prior evictions, income and other factors that are immediate obstacles to renting a house or an apartment. Goal 3 Measures: ? Average length of time people are homeless ? Percentage of households who receive housing services who do not become homeless again within 12 months ? Overall number of people in shelters ? Number of landlords and the number of units of housing that participate in the housing program ? Number and percentage of households who are assed for their barriers to housing within 7 days of shelter entry
