Award Description: Elkhart, Indiana, with its RV manufacturing turndown has become the city with the highest unemployment rate in the nation. With this IDS funding, the health center will respond in an appropriate manner to meeting the health care needs of this population by increasing the amount of available medical services to meet the most pressing problem for patients: access. Heart City Health Center (HCHC) will implement the IDS expansion through the addition of a .6 FTE IM locim tenens physician in year one, a 1.0 FTE NP in the second year and a 1.0 FTE medical assistant in both years - for a total of 2.6 new FTE's added. Due to difficulty HCHC has had recruiting physicians, locum tenens was selected as the method to best get services quickly. These providers will increase the amount of direct medical services to a number of life cycles : prenatal, pediatric, adolescent, adult and geriatric. In the first year, an IM physician will be available immediately to address the growing adult chronic illness patients. This will enable a wide range of outpatient services to patients with chronic illnesses who lack insurance and access to care. These patients will then also be eligible for Section 340B drug pricing, provided by HCHC's in house pharmacy. This strategy will enable the expansion of medical providers to increase access to care for the target population. Total encounters are projected to be 7,920 with 6,336 visits being delivered to the uninsured. This will provide a much needed increase of access to the uninsured. Total new patients are projected at 750 users. Hours of operation are not being extended at this time as HCHC has sufficient clinical delivery space to accomodate the increased provider without incurring additional overhead from expanding hours at this time. HCHC will conduct marketing to create awareness in the community about the increased services available.
Project Description: Heart City Health Center has seen 1,199 unduplicated new patients in the first 5 months of IDS reporting which resulted in 2,019 encounters. We have also noticed a significant number of patients who have lost insurance during this time period. For the the purposes of our reporting, new patient count began with the hiring of the locums physician on 4/29/09.
