Award Description: ARRA - Increase Services to Health Centers
Project Description: Through the IDS funding, Choptank Community Health System, Inc. (CCHS) anticipates improving access to health care services for patients in the service area by expanding patient care hours, increasing provider staff time, and expanding enabling and outreach services. The number of new patients resulting from these efforts is projected at 1,500 with an estimated 6,565 new visits. In the second quarter of the IDS grant (July - September 2009), 809 new patients were seen by CCHS medical providers for 1,364 visits. Of these patients, over 47.5 percent were uninsured, 40 percent minority, and 68 percent were adults under 65 years of age. Since March, a total of 1,360 new patients have been seen for 2,159 visits. Over 600 of these patients (45%) were uninsured. The CCHS Patient Services Program was developed utilizing IDS funding. In the last quarter, the Patient Service Coordinators (PCS) saw 223 patients for 319 enabling visits. The PCS? provided enrollment and support services for pharmacy, medical, and other social assistance programs. As a result, 15 percent of the patients seen by the PCS staff in the past quarter were enrolled in medical assistance programs (MA or PAC). In this past quarter, the IDS funding assisted to expand medical provider and outreach hours in the CCHS Migrant Health Program. To date, 500 migrant patients have been seen for over 1,600 visits.
