Award Description: ARRA - Increase Services to Health Centers
Project Description: MLMC will implement an outreach project targeted specifically to those persons that have been adversely affected by the economic recession. The project will be staffed by a health care resource advocate and a Nurse Practitioner. The resource advocate will conduct outreach in the community including direct contact with state and private employment assistance programs, with all businesses large and small, including all other public means to access the targeted population. The resource advocate will provide health literacy, which is the degree to which individuals have the capacity to obtain, process, and understand basic health information and services needed to make appropriate health decisions which will result in increased patients accessing services at MLMC. The staff will provide high quality service to persons that will be receiving medical care at MLMC by personally providing ongoing health status monitoring in conjunction with all of our providers and other needed health care services, and to also link them to other available resources in the community and statewide, specifically assisting with Medicaid eligibility as appropriate. With the addition of the NP to the provider pool at MLMC we will be able to provide increased access and the flexibility to add hours to the schedule to meet the demand. The NP will be the primary resource for providing services to this targeted population; however, patients may elect to receive services from MLMC other providers.
