Award Description: This grant will be used to implement Electronic Health Records (WorldVistA) for all Family Health Centers of San Diego medical and mental health practices. The grant will be used to equip Family Health Centers of San Diego with the technology infrastructure to support the implementation of WorldVistA opensource EHR, provide the human capital to effectively resource the implementaion and support the networking required to adopt best practices from other CHC implementations. Family Health Centers of San Diego will employ the following measures related to its deployment of the proposed EHR implementation: The 5 year project goal is that 90% of children, who are regular patients at FHCSD, will be fully immunized, based upon the Centers for Disease Control recommendations for vaccinations by the age of 2 years. Diabetes Control: The baseline FHCSD will work from in the context of this measure is 27.l% of diabetics with poor control (HbA1c > 9%). FHCSD is setting the following expectations related to this indicator over the next five years: No more than 26.5% of patients with either Type 1 or Type 2 diabetes will exhibit an HBA1c >9% at 12 months post-EHR implementation, with a reduction to 25% at three years post-implementation, and 23.5% at five years post-implementation. Early Entry to Prenatal Care: Drawing on its 2006 baseline, FHCSD provided these services to 1,938 women, of whom 68.5% entered care in the first trimester. FHCSD is setting the following expectations related to this indicator over the next five years: A minimum of 71% of perinatal patients will enter care within the first trimester at 12 months post-EHR implementation; a minimum of 74% will do so at three years post-implementation; and a minimum of 76% will do so at five years post-implementation.
