Award Description: The purpose is to increase the number of staff, expand hours at the HOMES fixed site location and increase mobile unit service sites.
Project Description: The grant allowed for the purchase of a personal computer for a nurse and a laptop computer for field access to the hospital system's electronic health records. Funding was used to send an employee to the Health Care for the Homeless Annual Conference. The nurse practitioner saw 107 new patients and provided care in 567 visits.
