Award Description: Community Health Partners will use the resources obtained through the Stimulus funding to support and expand organizational operations in the areas of medical care, behavioral health, and dental access. The IDS funds come at a perfect time and essentially all changes will be initiated within the first 60 days of receipt of funds! The organization has three medical sites. Each medical site will be positively impacted in terms of patient access to medical and/or behavioral health services. At the Livingston clinic, the Behavioral Health position will be expanded to be available at all times that the clinic is open - 40 hours/week. Funding also supports the addition of a mental health group visit session, an evidence-based approach to improving mental health outcomes. Similarly, the Bozeman medical site (Gallatin Community Clinic) will be able to add a 0.4 FTE behavioral health position to more fully meet client needs using an integrated behavioral health model (part of the medical visit). In Belgrade, CHP's newest site with increasingly high volume, we will be able to add significant medical capacity. This includes a half time Physician Assistant, her support staff, and again, expanded behavioral health staffing. This is making a huge difference at that small site, increasing access by 50%. Community Health Partners has two full time 1.0 FTE dental sites: Livingston and Bozeman, only one of which was directly impacted by IDS funding. The funds are being used to retain the full time staff in Livingston where there are no operating funds available. CHP opened this site through fund raising because the need for care was so great, but without ongoing funding. The plan continues to be to apply for BPHC funding for oral health. Twice we have been turned down. IDS funding is critical to keep this site open and will be used to support the costs of the dentist, the hygienist, the dental assistants, the coordinator and the front desk person, a total of 6 positions! Outside of medical, behavioral health and dental, CHP will also be able to add a human resources assistant position. This helps the organization, with its 120 staff members, function well!

Project Description: To date, using IDS funding, CHP has expanded behavioral health access in its Livingston, Bozeman and Belgrade medical clinics with a total of 1.0 FTE added. We have increased medical staffing so that effective October 1, 2009, an additional .5 FTE NP and staff are providing services in our Belgrade clinic. We have expanded case management services in Bozeman by hiring an additional .8 FTE. Additionally, we have added assistance in our HR department with .4 FTE there.
