Award Description: Increase service to health centers
Project Description: The Center had its grand opening ribbon cutting on July 14th 2008. The event had about 200 attendees; we had the privilege of having the Queens Boro President Helen Marshall, Senator Hiram Monserrate, Assemblyman Jose Peralta, and Council Member Julissa Ferreras. In addition we had the CEOs and President Michael Dowling from the North Shore-LIJ Health System, Chris Constantino Executive Director of Elmhurst Hospital Center, many leaders of the CBO, and community members enjoyed an indigenous ritual dance performed by the community youth. One of the main goals is to increase visibility in the community we participated in 12 health fairs and have held presentations at 4 children day care center in order to help increase the pediatric visits. In addition, we have partnered with several manage care companies to have hours at the center to help families enroll into insurance program or answer any of their questions. As per our mission to be a part of the community we have offered the use of the conference room to be used for local community meetings. We have a total of 4 small groups that monthly meet at the center for their workshops and board meeting. We also took part of Elmcor Youth and Adult Activities Day to prevent free obesity in our children, and sponsored a Youth Outreach and Education Day at the Center. We partnered with two health plans to promote a back to school and summer safety tips which bicycle helmets were given out. All events were held at the center and all individuals were given tours of the health center and were able to meet the health care providers during these events. This results in the best way for the community to get acquainted with the services we provide and the hours of operation.
Jobs Summary: We retained 4.2 staff ftes including pediatricians, case managers, management and enabling services. we also created .89 new staff fte including nurses, MA, and case managers. (Total jobs reported: 5)
