Award Description: Recovery Act Health Center Cluster Program: North Fork Valley Community Health Board and the University of Kentncky (UK) College of Medicine as co-operators of the North Fork Valley Community Health Center (NVFCHC) are seeking a Section 330 satellite new access point grant to establish a health care center (June Buchanan Medical Clinic) in Knott County that will offer quality health care to those in need in Southeastern Kentncky. The co-applicant arrangement will continue for the operation and oversight of this New Access Point. The proposed health center will operate from the conversion of a rural health clinic previously managed by Appalachian Regional Healthcare, which had been used for direct medical care in Knott County. The Knott County Satellite New Access Point will offer new services to our adjoining county, in accordance with our vision of becoming a vital and responsive community fixture in the health care landscape of Eastern Kentncky. This new satellite access point will provide quality health care through an integrated system of health care providers. Services will include comprehensive women's health care encompassing obstetrical care and family planning, pediatric acute and well child care, newborn care, geriatric care, and immunizations for both children and adults. The health center will also offer a range of services like physicals for preemployment, sports physicals, and workman's compensation exams. Supplemental services will include case management, health education, low-cost pharmacy and transportation assistance. The community health center will also provide mobile oral health services through use of an mobile dental van (for the pediatric dental population) and a fixed dental site (to serve the adult population). Mental health services will also be available on-site. Community outreach, community health education and a range of collaborative efforts with other local agencies will be added to this array of services to form a comprehensive system of health care and support.
Project Description: The University of Kentucky North Fork Valley Community Health Center has established a satellite new access point at June Buchanan Medical Center, a rural health clinic previously managed by Appalachian Regional Healthcare. The satellite new access point currently offers primary medical care, pharmacy, and dental services at our fixed site clinic. While a dental mobile for the Knott County population is not available as of this writing due to lack of funding for a mobile program, the dentist at June Buchanan does operate an outreach program for area elementary schools. The medical clinic also offers expanded hours to increase accessibility (the medical clinic is open from 8:00-6:30 Monday-Friday). Roughly half way through our initial year of operation (March 1-September 30), we have enrolled 561 new dental patients and 1150 new medical patients (of which 1667 are unduplicated users) in 4,746 encounters. Of the 1667 unduplicated patients served, 20%, or 341 patients, were uninsured. Our first year projections (entire year) were for 3600 users (of which 2700 would be unduplicated) and 9,700 total encounters. According to these projections, we have exceeded our estimate of unduplicated users in our first 7 months of operation, while our encounters are slightly below initial projections. We anticipate that given sufficient patient enrollment, we should increase encounter capacity during our typically busier fall and winter seasons. We have also recently hired a psychologist who will devote 25% of his time to June Buchanan Medical Clinic, and that position will likewise contribute to encounter capacity during the next five months.
Jobs Summary: The new satellite access point currently employs two physicians (2 FTE's), a nurse practitioner (1.0 FTE), a nurse midwife (.2 FTE), a dentist (1 FTE), and support staff {nursing (3.0 FTE), medical records and clerical staff (4.0 FTE), laboratory staff (2 FTE), dental (2 FTE), pharmacy staff (3 FTE) and facility staff (.5 FTE)}. This complement of 4.2 FTE providers' and14.5 support staff (for a total of 18.7 new FTE positions) exceeds our 1st year grant projections of 4.0 FTE providers and 12 support staff. We have also tendered an offer for our proposed Operations Officer, who will be instrumental in implementing our CIP activities (not yet started) at June Buchanan Medical Clinic.
