Award Description: Volusia County through its Community Assistance Division will implement the HPRP and directly provides services to the program participants. HPRP staff will provide program training and technical assistance to the Continuum of Care (CoC) partner agencies and other agencies that will serve as referral sources to HPRP. Individual and families who are 'at risk' of becoming homeless and need temporary assistance will be identified through Community Assistance staff and referred to the HPRP case manager for a consultation, assistance focused on housing stability, and linkages to community resources and mainstream benefits. To assist individuals and families who are experiencing homelessness (residing in emergency or transitional shelters or on the street) and need assistance in order to obtain housing and retain it, a referral process will be implemented through Memorandums of Understanding (MOU's) with the CoC partners to the HPRP case manager. The HPRP case manager will collaborate with the referral agency's case manager to develop a re-housing plan. The plan will include the types of assistance needed to transition the participants to independence and permanent housing stabilization.
Project Description: The Voulsia County HPRP framework targets housing for two eligible populations: the 'at risk' of becoming homeless (Homelessness Prevention) and those who are homeless (Rapid Re-housing). Homelesness Prevention unit will provide assistance to individuals and families who are currently in housing but are at risk of becoming homeless and need temporary assistance to prevent them from becoming homeless. Approximately 35% of the HPRP funds will be allocated to provide the following assistance and housing reclocation and stabilization services:Case management, Short and medium term rental assistance, Security and utility deposits, Utility payments, Moving costs assistance, Motel/hotel vouchers. Rapid re-housing unit will assist indificuals and families who are experiencing homelessness (residing in emergency or transitional shelters or on the street) and need temporary assistance in order to obtain and retain housing. Approximately 65 % of HPRP funds will be allocated to provide the following assistance and housing relocation and stabilzation services: Case management, Short and medium term rental assistance, Security and utility deposits, Utility payments, Moving costs assistance, Motel/hotel vouchers.
