Award Description: The overall purpose of the electronic health record (EHR) project (Alabama Healthcare Consortium Network EHR Implementation) is to support the Health Center Controlled Network (HCCN) known as the Alabama Healthcare Consortium Network (AHCN) to implement an electronic health record system (EHRS) as a tool to improve the quality of health care provided in their four health center member organizations and to achieve improved individual and population health outcomes. The proposed project creates a sustainable business model of EHRS implementation and health information technology (HIT) integration to provide lessons learned for building capacity through strategic partnerships and positive participation in pay for performance. The total number of organizations impacted by this proposal are 4, of which 4 are Section 330 grantees. The total number of sites impacted by this proposal are 36, of which 36 are Section 330 grantees. The total number of patients impacted by this proposal are 99,552, of which all 99,552 are Section 330 patients. The AHCN service area consists of 21 counties in west, central, and south Alabama and includes some of Alabama?s most economically depressed counties, some of the highest chronic disease rates, and highest poor health risk factor prevalence in Alabama. Risk factors for these chronic diseases are often shared by more than a single diagnosis, therefore co-morbidities are common. For example, in the service area, attempts to reduce the number of deaths due to heart disease are complicated by a high incidence of diabetes. The project employs an on site implementation team including clinicians, Information Technology staff, and others representing each health center. In this manner, the project will achieve key change management and EHR implementation according to schedule. Through the acquisition of a chosen EHR product, a single vendor and platform will be achieved for integrating functions and sharing data to improve health center operations and maximize resources. The chosen system has the ability to generate key data to measure individual and population health outcome changes. Overall, the project will: (1) increase health center capacity to measure and report health care quality outcomes, (2) use HIT to reduce health center costs, (3) improve patient access to and use of health care data and services to increase their participation in their own health status, (4) improve point of care decisions, and (5) improve health center and Network sustainability.
Project Description: The Alabama Healthcare Consortium Network (AHCN) received notification of its award on September 29, 2009. Given the short time frame since that notification, activities are in the most preliminary stages. The Network Board of Directors has met to address the following priority actions: 1. Full implementation of the Network staffing; 2. Finalizing vendor negotiations; and 3. Resolution of the one (1) Notice of Grant Award special condition - detailing the federal vs. non-federal budget.
