Award Description: This award is being used to expand the primary care services provided by Advocates For A Healthy Community, Inc. d/b/a Jordan Valley Community Health Center ('JVCHC'). Prior to this award, JVCHC hours of operation were Monday through Friday, 8:00 am to 5:00 pm, and our medical schedules were filled to capacity. With this award, we were able to hire an additional nurse practitioner, two in-take workers and one nursing staff. These additonal staff assisted us in the expansion of our clinic hours to Monday through Friday, 8:00 am to 10:00 pm and Saturday, 8:00 am to 5:00 pm. The expanded hours are allowing us to see an estimated 720 extra patients a month, providing a safety net of medical service to those who otherwise may not receive it.
Project Description: The overall purpose of the award was to expand our clinic hours to Monday through Friday, 8:00 am - 10:00 pm and Saturday, 8:00 am through 5:00 pm. These hours would provide an additional 28 clinical hours per week into our existing schedule and allow us to see an estimated 720 extra patients a month, providing a safety net of medical service to those who otherwise may not receive it.
Jobs Summary: Four individuals were hired with these funds, one nurse practitioner, two in-take workers, and one LPN. The hiring of these individuals assisted us in the expansion of our clinic hours. All four individuals were hired full-time. In addition to these four employees hired with the grant funding, an additional phone scheduler, security guard, pharmacist and lab technician were hired at the expense of JVCHC to provide the needed medical services during these hours.
