Award Description: We have successfully recruited and hired a full time Psychiatric/Mental Health Nurse Practitioner who started August 1, 2009. She can now start to build a patient base and provide comprehensive assessments. By addressing the patient concerns and making confident disposition recommendations to the patient's primary care provider. Outcomes will be measured by providing screening tools that are culturally competent and appropriate for varying literacy levels. We can ensure that screening takes place across entire spectrum of development. Incorporating behavioral health into quality improvement programs will be another way to measure our project's outcome.
Project Description: This project addresses the community need by allowing us to hire a full time Psychiatric/Mental Health Nurse Practitioner. By adding this provider we can now direct our patients to an onsite behavioral health assessment and treatment. It will also benefit those patients who have other chronic health problems whose co-morbid psychiatric conditions result from, complicate, or interfere with the other health programs or their treatment, e.g., difficult adjustments to diabetes. In addition, care monitoring and chronic disease management, which can be effectively managed in the primary care setting, such as less complicated cases of depression.
