Award Description: Family Health Centers of San Diego received a New Access Point grant from the Health Resources and Services Administration to establish a new community health center in Lemon Grove, CA. By the 3rd year of the Lemon Grove Family Health Center’s operations (first full operational capacity year), FHCSD expects that this center will serve 4,522 unduplicated patients for a total of 11,757 encounters. Lemon Grove Family Health Center will be a comprehensive primary care site based on a freestanding, fixed-site model. Services to be provided at the Lemon Grove Family Health Center by the conclusion of the 2nd service year will include: general primary medical care; waived laboratory services; urgent medical care; family planning services; HIV testing services; immunizations; OB/GYN care; TB therapy; mental health services, including counseling and limited provision of/referral for substance abuse treatment; hearing screening; nutrition services; vision screening; referral for comprehensive oral health care; case management; eligibility assistance; health education supported by outreach and linkage activities throughout the targeted service area; and translation and transportation assistance.
Project Description: Family Health Centers of San Diego (FHCSD) was awarded a New Access Point (NAP) grant to establish a health center in Lemon Grove, CA. The originally proposed site was no longer available when the award was made. FHCSD worked quickly this spring to identify a new location just three blocks from the previously proposed site. For long-term stability, FHCSD decided to purchase rather than lease the site. FHCSD closed escrow on the site on July 31, 2009. Renovation plans were developed and submitted to the City of Lemon Grove’s Building Department in August of 2009. In addition, FHCSD obtained a competitive $150,000 grant from the Grossmont Healthcare District to fund a portion of the construction work. FHCSD received a building permit for the site in early September, and construction commenced immediately. Demolition is complete and interior renovations are now well underway. All construction work is projected to be completed by December 15, 2009. FHCSD will equip the clinic in the last two weeks of December and will open Lemon Grove Family Health Center during the first week of January of 2010. Key clinical staff have been selected for the new Lemon Grove Family Health Center, including the Clinic Director, Charge Nurse, Medical Assistant, Certified Phlebotomy Technician, Medical Records Clerk, Patient Services Representative, and Referral Specialist. These individuals are currently training at other FHCSD clinical sites and will transfer to the new site once construction is complete. FHCSD has initiated a community outreach campaign to share information about the new clinic with community leaders, key community partners, and potential patients.
