Award Description: Aims of the proposed Competitive Revision: The proposed revision to the MeTeOR trial addresses the crucial question of whether APM accelerates structural progression of osteoarthritis compared to non-surgical management of meniscal tears. It also examines the clinical meaning of progression in various structural parameters. These objectives lead to the following four Specific Aims: 1. To obtain radiographs and MRI studies as well as questionnaire data (WOMAC pain and function scales, EQ-5D, transition questions that assess the subject?s impression of whether they have improved or deteriorated) from subjects at 18 months from study enrollment. 2. To conduct standardized reading of the radiographs and MRI scans using Kellgren-Lawrence grading for radiographs and the semi-quantitative Boston Leeds Osteoarthritis Knee Score (BLOKS) scoring system and quantitative 3-D cartilage morphometry for MRI. 3. To develop an integrated MeTeOR database that couples clinical data with a centralized repository of longitudinal (baseline and 18 month) radiographs and MRI studies in order to prepare for analyses of differences in rates of OA progression between the two trial arms (APM and nonoperative). 4. To establish the inter- and intra-rater reliability of imaging findings and clinical interpretation of changes in imaging parameters.
Project Description: Deliverables: 1. Obtain baseline radiographs and MRIs from MeTeOR centers BWH IRB application submitted and pending. 2. Obtain 18 month follow up radiographs, MRIs and questionnaires Will obtain these starting in January 2010, pending IRB approvals 3. Develop central database at Data Coordinating Center for these new data, to integrate with MeTeOR study data DCC beginning to plan data base enhancement to accomodate MRI data 4. Read the MRIs and radiographs centrally Pending obtaining these studies (after January 1, 2010) 5. Conduct analyses of intra rater and inter rater reliability and clinical interpretation of imaging parameters. Pending obtaining these studies (after January 1, 2010)
